I consent to IduScore processing my health-related questionnaire answers, blood-marker data, lab appointment data, and generated report data for the purpose of providing my IduScore wellbeing report, coordinating blood collection where included, supporting my order, and maintaining my report history unless I request deletion.

I understand that IduScore is a non-medical wellbeing product, is not a medical device, and does not provide diagnosis, treatment, prevention, disease management, or medical advice.

I understand that anonymized data may be used for product improvement.

What the Customer Consents To

The customer consents to IduScore processing:

Questionnaire answers.

Blood-marker data and related files.

Prior disease and family history answers.

Lab appointment and sample status data.

Generated wellbeing report outputs.

Support messages related to the service.

The customer also consents to necessary sharing with service providers and partners, including Wix/payment providers, Vitalcheck, Google Cloud Platform, OpenAI, and future at-home collection partners where applicable.

Important Limits

IduScore does not provide medical advice or diagnosis. Customers should contact a qualified healthcare professional for medical questions, symptoms, diagnosis, treatment, medication decisions, screening, or emergency support.

Withdrawal

Customers may withdraw consent by contacting:

Withdrawal may prevent IduScore from completing the report or continuing the service. Some data may need to be retained where required by law, accounting obligations, dispute handling, security, or fraud prevention.
